The most important thing is to know your medical history. This includes all your current medications, their dosages, all previous surgeries, and any problems with anesthesia.

Complete the patient history form enclosed in the packet. Bring this with you for your pre-testing appointment (if applicable) or on the day of your scheduled procedure. Please use the Admitting entrance located just past the Emergency Room entrance on the side of the hospital.

Plan to spend an hour with us if you are scheduled for a pre-testing visit. Lab testing, EKG assessment, and anesthesia workup may be completed during this time. Please bring your medication bottles with you when you come for pre-testing.

Bring a list of any questions or concerns you may have.

It is very important that your surgeon has current contact information on the day of your procedure -- this includes cell phone numbers and hotel information.
